Do I need a referral?

Generally, referrals are not required to obtain mental health services. HCPA will assist in obtaining referrals if required by your insurer. You can help by letting us know when your insurance company or health plan requires a referral or prior authorization for specific procedures. If you have questions about referrals, call your insurance provider for information and assistance.

What are my rights and responsibilities?

At Harbor City Psychological Services we believe in thoroughly assessing your needs and working with you to develop a plan to reach your goals. We believe that it is important that you understand your client rights, responsibilities, and obligations.

Voluntary Participation

All services provided at Harbor City Psychological Services are voluntary services and you may choose to discontinue services at any time. Your consent for participation in services will be requested at the initial interview. For children under l8 years of age, consent for participation in services is required from the parent or legal guardian before beginning services.

Confidentiality

Services provided by Harbor City Psychological Services are considered confidential and fall under the Heath Information Privacy Accountability Act (HIPAA). Therefore, information shared cannot be released without your written authorization and consent unless it meets one of the criteria indicated below:

  • There is reason to suspect the abuse or neglect of a child, vulnerable adult, or elderly person. As mandated reporters our clinicians are obligated by law to report suspected incidents of abuse or neglect to the proper authority.
  • There is risk of imminent harm to yourself and disclosure is necessary to prevent the threatened risk of harm or danger to self.
  • Risk of imminent harm or threat to another person and disclosure is necessary to prevent the risk of harm or danger to another person.
  • Court Order to release information.

What forms of payment do you accept?

Private Insurance:

We accept most insurance plans and will work with your insurance to verify eligibility of services prior to your first visit. To inquire about your Mental Health benefits you can consult member services. The phone number is typically on the back of your insurance card. Our staff will also work with you to ensure you understand the benefits, including your obligation for co-pay and deductibles if they apply. Clients are ultimately responsible for payment of their account balances.

Public Insurance:

We are providers for Minnesota Medicaid plans, including those administered by commercial insurance companies. Certain clinicians are providers for Medicare.

Cash Fee for Service:

Please contact our office to inquire about our fee schedule if you will be paying cash. We do offer a cash discount for services paid at the time of service. Our staff can inform you of these rates.

After-Hours/Emergency Care

When our offices are closed, clients are asked to call the Essentia Health Crisis line at 218-723-0099 or 911 in emergency.
